Enrollment Process
Dependent Coverage:
- Copies of Marriage Licenses and Birth Certificates are required for dependent claims but not for initial approval or enrollment.
- Lost your certificates? Contact us for guidance on obtaining replacements at low or no cost.
- Online Autopay is Required: If application is submitted without banking info, application will not be approved.
- Final Step: Once you’ve completed your Ease enrollment application, email us at info@peo4you.com to get confirmation ASAP.
Important Notes on 12-Month Enrollment Minimum
- Enrollment in the plans is for a minimum of 12 months. After 12 months, enrollment is month-to-month.
- You are allowed to remove dependents prior to 12 months.
Important Note on Preventing Confusion at the Doctor’s Office
- The Blue Cross logo is printed on both the front and back of the cards you will receive. Be sure to tell your providers to call CSJB and the numbers on the card so that they do not call Blue Cross and tell you that you do not have coverage.
